To make a referral to us you can contact us directly. A family member of advocate can also make an enquiry on your behalf. We will come to your home and assess your personal needs, your internal and external environment so that you and your carers are not put at risk. Following this a personalised care plan will be made and you will be central to this process. Once the service is agreed we will appoint carers who will become keyworkers who then carry out the care plan. After a settling in period of about two weeks or sooner if necessary, we will review to establish you are satisfied your needs are being met to a high standard.
If you are in hospital or other institution and you are being discharged and need care for the first time, the hospital has a duty to ensure you are safely discharged home. They will contact the social services hospital discharge team (or similar) to assess your level of need. If you qualify for support they will arrange the care or they may tell you how to access care if you are funding it privately.
In some circumstances the NHS pays for care. You should see the hospital discharge management to have this assessed. They will inform you of the terms and what conditions apply.
